Student Medical and Health Forms
All new and returning Trinity families must complete required student health and medical forms through Magnus Health, our new secure health management system integrated with Veracross.
Important Dates
June 4 - Magnus Health is available through Veracross Parent Portal
June 15 - Deadline to submit all required health and medical paperwork
August 31 - All forms must be completed before students begin school

What to Expect
Complete health information for each enrolled child.
The system will identify the specific forms and requirements for your child(ren).
Allow approximately 20-30 minutes per child to complete the required forms .
After the initial setup, future updates will only require your review and update of information on as needed basis.
Families will receive weekly reminders until all required health information has been completed and submitted.
